Hello doc , if an adult patient permanent molar tooth was treated with silver diamine fluoride before, and now the tooth looks stained. Will an indirect inlay/onlay ceramic still be a good treatment option? Will there be bonding issues? Or not indicated? Thank you
I won't be an issue - clean the area with air-abrasion or cover with RMGI, then build up to ideal contour and prep ideally.

If you see any visibility of the pulp from a crack tooth, it's time for a root canal?
Yes, it certainly could be but a pulpal and apical diagnosis will be required. Cracked teeth without direct pulpal involvement present high risk restorations, so one with a pulp exposed would be a time to proceed with caution...

I had few composites done on my molars. Wasn't informed about the longevity and disadvantages. And one tooth is giving me pain while biting down. Do dentists in India practice this kind of inlays? Want a permanent long term solution

Its only a shame that so many dentist, are able to become dentist even though they are not good enough to produce quality work. You watch these videos and think this is the work you will get. But for some reason, there are many dentists who do really bad work, and yet are still able to stay dentists doing bad work. Only if they all did good quality work like we see in these BS videos lol.
Doctor, please answer my question. I am a fourth-year dental student. When I get the filling after about a week or less, the filling falls out completely, even though the materials used in the filling are good (I use the Tokiyama filling).The bond is also good, and the cavity I make has the same conditions required for an amalgam filling cavity, but I do not know what the main reason is.
Could be contaminated air - oil in the system. This would be the only thing I can think of other than moisture contamination, but I doubt this is the issue.

hey, needing advice! I am retaking and had the 29 DO and I did not leave any caries but I over-prepared my procimal wall, axial, and isthmus. i was confused because i thought they wanted us to go to the max of each of these dimensions ie near 1 mm proximal clearance, 1 mm from the DEJ etc. but also, there was mesial occlusal caries that you could not find unless you went 2mm deep and some people faield because of left caries in that area. is it typical to have the caries not connected for this exam? any adivce on going to the max i would appreciate!
I would prep to ideal then only extend those walls required to move due to caries, slightly less than the ADEX max allowed, then involve the FE for permission to extend. If you prep 1.5 mm pulpally, you can see the decay. Stick to a traditional style prep, and not a slot or box.

This is modeling resin, but it's not really required and I usually don't need it. I use a very nicecomposite that will not stick (Tokuyama Esteelite Omega) and a Titanium Nitride plasma coated instrument.
If there is old composite on gingival margin to do DME and looks ok should I remove it to do DME myself before making onlay or should i leave it? I always thought the best is to have no compost or glasjonomer on margins of preparation?
Great questions - you are correct. When making an indirect restoration that is luted (non-adhesively), the ideal would be to have only one material at the interface, however, with a bonded procedure the substrate needs to be considered - in a gingivally extensive case, the substrate is root surface, which is both difficult to isolate and presents us with a challenge for long term seal. The DME (or more accurately GEME) allows us to focus on establishing a clean and optimal bond and a raised margin which may then be sealed in a different step. It all boils dow to access and isolation.

What bur I need to use to get a chamfer margin of width between 0.3 to 0.7 doc? And can I use half of the round end taper bur or need to use only torpedo bur? Please advise. Thank you in advance
The 877-010 is a parallel pointed diamond 1.0 mm in diameter and this will give you a 0.5 mm modified chamfer. If you would prefer a tapered chamfer bur, use the 878K-016 which will give you a 0.55 mm chamfer if using half of the tip diameter. Round ended diamonds don't give chamfer margins - they create fillet finish lines, which are more like a rounded shoulder.
Dr. How far(millimeter) from the occlusal surface do you place that palatal shelf for onlays? Thanx!
The shoulder may be anywhere it needs to be based on the quality of the remaining structure. Usually in the most conservative situations, it resides at the base of the A plane (Functional Cusp Bevel), or approximately level with the pulpal wall.

Wow, this was done on my tooth #3 YESTERDAY for a 3/4 palladium crown. Wearing a temporary as we speak. No idea the prep was this sophisticated. I feel fortunate to have found her-a practitioner with Ph.D in Prosthodontics from a UK university-cause I live in the US and she practices in Southeast Asia. Had a trustworthy referral. Dr. Stevenson, thank you for the video. How come the internal axial wall of the proximal box is concave, not convex? Shouldn't we be concerned about the pulp proximity?
The axial depth is still ideal, it's just that the axioproximal line angles are rounded...the idea being that the prep is caries-driven with composite. The pulp is 3-4 mm inside the box. The axial depth would be the same for any preparation, namely about 0.5 mm into dentin. Thank you!

Thank you for watching - you have the best attitude! I have always been slower than others and I don't worry. Slower is better than faster with less refinement. You will speed up over time...years of practice.

I have a query. When we are cutting through a diagnostic mock up for veneer preparation, all the composite resin left behind s remove n then an impression is made or before that.
Remove all of it, roughen any un touched enamel, and create a finish line, then take the impression. It is key to try to make a prototype mock up and not strictly an additive mock up. Instruct your lab to prepare the teeth prior to the wax up to place the proposed contours in the precise location desired. Often the putty made from this will not fit onto unprepared teeth, hence the need to prep them first.

I would use resin cement and a fully bonded protocol for these types of preps. With full coverage, and adequate retention and resistance, I'm comfortable with RMGI.

Had a root canal yesterday, and all day I couldn’t figure out why my gum margin was sore. Never had a dental dam used on me before, so I figured that be the logical thing to look up. That clamp/clip looks like it makes total sense, why my gums are sore. Oh well, so far that has been the worst part of getting a root canal, and it’ll probably be better in 2-3 days.
Yes, and the type of clamp makes a big difference - if the dentist uses conservative clamps, no pain or soreness after...

Yes, this is great. If you do lay done the first areas in lobes, with a dentin shade, the results could be better, shade wise, but this is optional.

From what I've heard, simpler approaches can sometimes give good results, but it really a case to case basis. It's still best to get expert opinion

A well reconstructed class IV composite may last many years - often 15-20 years or more. After several years they are prone to chipping and lose some of their initial polish, but may be repaired easily. This would be my first choice - a conservative composite. If it fails prematurely, it may be necessary to have a veneer or crown made. Remember: the most important variable is the skill and experience of the dentist, not the procedure itself...Best wishes, Dr. S
